LIVING A HEALTHY LIFE WITH CHRONIC CONDITIONS
The Chronic Disease Self-Management Program (CDSMP)
- "Living a Healthy Life with Chronic Conditions" is a free health education workshop offered by University of Victoria, Centre on Aging.
- Pairs of trained volunteer leaders deliver the six-session workshop to groups of people who have chronic health conditions and their caregivers.
CDSMP VOLUNTEER LEADER TRAINING
- You do NOT need to be a health professional to lead the workshop. Many of our volunteer leaders have chronic conditions themselves.
- To become a certified volunteer leader, you must attend the four-day Volunteer Leader Training.
- The leader training and the detailed Leaders manual are provided at no cost.
- As a volunteer leader, you will be able to use the skills you have learned to help people help themselves to manage their daily challenges.
